Locker Items Return Form

We would like to provide guidance on the process to pickup any personal items and/or valuables that remain in student lockers.


In an attempt to minimize the flow of people and adhere to the continued guidance for distancing, we have developed a procedure to reduce contact. Please complete the Locker Items Form in the link below if your child has personal items that he/she needs to retrieve from their locker.

Once you complete the form, you will be contacted by the building principal with a date and time to retrieve these belongings. If you do not complete a form, the district will assume your child does not have any items that need retrieved. Therefore, his or her locker will be emptied and all items discarded.
